He was predeceased by: his parents, James Fornwalt and Brenda Fornwalt (Deaton). He is survived by: his sister Karen Fornwalt of Ypsilanti; his significant other Jolene Kovacs of East Pointe; and his friend Brent. He is also survived by several aunts, uncles, cousins.

Contributions in his memory may be made to Michigan Humane Society.
