He is survived by: his father Otis Holm; his son Thomas Fairbanks (Emily Kadlec); his siblings, Craig Holm (Lisa Haberman) and Cindy Hanson (Brad); his nieces and nephews, Nathan, Allison Holm and Meigan Wills (Eric); and his great-niece Olivia.
