He was predeceased by: his parents, Joseph Kozisek and Emma Kozisek (Chalupsky); and his wife Gwendolyn Karen Hotzler. He is survived by: his daughters, Mary Klages and Sue Cook. He is also survived by two grandsons; two great-grandchildren.
