He is survived by: his parents, Zachary and Liz; his brothers, Forrest and Boone; his grandparents, Carolyn Hazlett of Fayette, Tom and Mindy Lockwood of Harrisburg; and his aunts and uncles, Lizzy Phillips, Jena Hazlett, James, Jay and Justin Hazlett.
