He was predeceased by : his parents, James Byron Lewis and Lou Ellen Lewis (Beeman); his brothers, James Lewis of Salem, Missouri and Ronald Lewis of Marshfield; his brother-in-law Terry Cook of Salem, MO; and also Wayne Lowe.

He is survived by : his wife Vicky Lewis (Cook); his son Anthony Lewis (Cassie); his grandchildren, Oliver Lewis and Abigail Lewis; his brother Jerry Lewis (Gladys); his sisters-in-law, Gail Lowe, Karen Lewis and Cindy Patrick (Edward); and also, Amy Lewis, Chris Lewis (Theresa), Amanda Lewis, Scott Lewis (Lauren) and Virginia. He is also survived by nieces and nephews.

Burial will be at a later date in Round Pond Cemetery, Salem, MO. For those desiring, memorial contributions may be made to the American Heart Association and left in care of Fraker Funeral Home, Inc., P. O. Box 85, Marshfield, MO, 65706.
