He was predeceased by : his parents, Gary Lewis and Myrtle Pulley Lewis. He is survived by : his wife Patricia "Pat" Sullivan; his daughter Callie; and his siblings, Sheri Lewis and Ricky Lewis. He is also survived by loving family members and friends.
