He was predeceased by: his daughter Nancy Lee; his mother A. D. Mustapha; and his sister Dorothy Libbee. He is survived by: his wife Sherry Mustapha (Wells); his niece Dannette Libbee of Springfield; his brother-in-law Ross Wells (Pat) of Wildwood; his sibling's children, Michelle Farmer (Tom) of Wildwood and Scott Wells of Ann Arbor; his great-niece Karen Farmer; his great-nephew Mark Farmer (Lauren); and also, Pam, Katie, Dezo and Renee.
In lieu of flowers, memorial contributions may be made to First Baptist Church of Kansas City, Missouri in Jerry's name.
