He was loved and cherished by many people including : his wife Mary Ann Kohoutek; his children, Cindy Gauthier (Jim), Jay Kohoutek, Patrick Kohoutek, Mollie Metzger (Glenn) and Susan Ludwick (Tom); his grandchildren, Jennifer, Matt Gauthier (Megan), Brandon (Kay), Kyle, Sara and David Metzger; his great grandchildren, Jack and William Gauthier; his siblings, Jerry Kohoutek (Annette) and Joe Kohoutek (Charlene); and his parents, Frank "Chody" and Hazel Kohoutek.
In lieu of flowers, contributions may be made to The ProMedica Hospice Memorial Fund, formerly Heartland Hospice Memorial Fund or the Alzheimer's Association.
