He was predeceased by: his mother Barbara Mitchell (Cook); and his grandparents, Andrew, Lois Cook (Hanson), Lesley and Katherine Waller.
He is survived by: his siblings, Lisa Waller (Joe) and Michael Waller; his aunts and uncles, Richard, Mona Strauser, Donald and Vicki Waller; and his nieces and nephews, Ashley Givens (Josh), Lauren Knoblauch (Zac), Coleman Beckerle (Ashley) and Shelby Webb (Michael). He is also survived by great nieces, nephews, cousins and friends.
