He was predeceased by: his parents, John Wesley Sullivan, Sr. and Mary Clay Sullivan; and his grandparents, S. O. of Louisville, Jewel Clay of Louisville, Bill of Louisville and Bertha Sullivan of Louisville.
He is survived by: his wife Stacy Ivy Sullivan; his children, Jaycob Sullivan of Louisville and Marybeth Sullivan of Louisville; his siblings, John Wesley "Johnny" Sullivan, Jr. of Louisville, Kenneth Clay "Kenny" Sullivan of Louisville and Mandy Trapp of Miami, Florida.; his nieces, Morgan, Jordan Sullivan and Lexi Holman; his great-nephew Jack Allen Sullivan; and his great-niece Mamie Jewel Phillips. He is also survived by numerous aunts, nieces, nephews, cousins, and friends.
