He is survived by: his significant other Aletha "Lee" Hall; his children, Penny Nelson (Allen), Dora Forehand, Gwen Caldwell (Joe), Scott Forehand (Hope), Kevel Felton (Amanda) and Heather Blackledge (Kyle); his grandchildren, Michael Hebert Jr., Charles Hebert, Todd Hebert, Caitlin Clatterbuck, Jared Caldwell, Skyler Capobianco, Devin Tillman, Trae Tillman, Scott Forehand Jr., Austin Forehand, Kyan Forehand and Braiden Tillman; his great grandchildren, Makinleigh Clatterbuck, Kai Caldwell, Anai Caldwell, Dorian Forehand, Todd Hebert Jr., Adlynn Hebert, Jase Hebert and Aubree Forehand; and his siblings, Versa Simmons (Bud), Michael Forehand (Karen), Samuel "Mutt" Forehand (Amanda), Paul Forehand, Charlie Forehand (Peggy), Roger Forehand (April) and Isaac Forehand (Michelle). He is also survived by many nieces and nephews.
