He is survived by: his wife Aimee Burleson; his children, Addyson and Reed; his parents, David Putnam and Denise Burleson; his grandmother Ophelia Blalock; his brothers, Kris Burleson (Karen), Nick Putnam (Hannah) and Jason Putnam; and his siblings' children, Cameron and Savannah Burleson.
