He was predeceased by: his mother Ruth Hairston; and his brother Eric Smith. He is survived by: his father William Polk (Berurene); his wife Regina Polk; his children, Kyle and Kendall Polk; his siblings, Waymon Smith, Earline Bryant, Sheila Hairston and Rhoda Hairston; his brother-in-law Clyde Bryant; his stepsiblings, Kevin Polk (Vicki) and Keisha Polk; and his brothers, Danny Bass, Warren Armstrong and Dave Daniels. He is also survived by aunts, uncles, nieces, nephews, cousins, and loving friends.
