He was predeceased by: his grandson Nathaniel Hooks. He is survived by: his wife Rena Helms Hooks; and his children, Kim Cartrett (Berry), Johnny Dale Hooks, Jr., Ronnie Lee Hooks (Mindy), Michael Shane Hooks (Ana) and Dustin Lee Hooks. He is also survived by one sister, two brothers, ten grandchildren and seven nieces and nephews.
