He was predeceased by: his parents, Carl M. Millard, Sr. and Mary Hinson Millard; and his brothers, Carl M. Millard, Jr. and William Millard. He is survived by: his wife Patricia Hollingsworth Millard; his sons, Mark Millard and Craig Millard; his brother Dan Millard, Sr. (Sharon); and his friends, Rhonda Daniels, Pat Wolford and Robert Fuller.
