He was predeceased by: his parents, Edward Joseph Dolezal and Anita Jo Stwalley Dolezal. He is survived by: his wife Elaine Dolezal of Hickory, NC; his children, Jessica Willis (Jacob) of Phoenix, AZ and Shane Dolezal-Liddell of Asheville, NC; his stepson Gabriel Boileau of Hopkinsville, KY; and his sister Stephanie Wilke of Paducah, KY. He is also survived by 3 Grandchildren and 1 Step-Grandchild.

In lieu of flowers, memorials may be made to the Wounded Warrior Project, P.O. Box 758516, Topeka, Kansas 66675-8516.
