He is survived by: his children, Cherie Smith of Hickory, Phillip Cook, Connelly Springs and Tina Cook of Atlanta; his grandchildren, Angelica Clark of Hickory, Preston Seitz of Hickory and Marcus Cook of Winston Salem; and his siblings, Johnny Cook of Connelly Springs, Robin Cook of Connelly Springs, Ronnie Cook of Connelly Springs and Kay Christopher (Ray) of Hickory. He is also survived by numerous nieces and nephews.
