He was predeceased by: his sister Frances Blickenstaff. He is survived by: his wife Rebecca Ferebee Pickett; and his daughters, Ellen and Elizabeth Pickett.

In lieu of flowers, the family asks that memorial contributions be made in Jack's name to charity of one's choice.
