He was predeceased by: his parents, Samuel Thomas Blackwell, Sr. and Sarah Juanita Lambeth Blackwell; and his brother Terry Blackwell. He is survived by: his children, Samuel Blackwell, III (Anna) of Archdale and Brandi Clark of High Point; his sisters, Teresa Williams (Kerry) of Trinity and Cynthia Hucks (Harold) of Gainesville, Georgia; and his grandchildren, Cammie Blackwell, Casen Clark and Montgomery Blackwell. He is also survived by one niece and three nephews.
