He is survived by: his wife Linda Vaughn Cook; his son William Brady "Brad" Cook (Kim) of Boise, Idaho; his brother Tommy Cook of Madison; his sister-in-law Dr. Elaine Montgomery of High Point; and his granddaughter Carleigh Cook.
