He was predeceased by: his parents, Arthur Cook and Marian Cook; and his brother Jim Cook. He is survived by: his wife Frances; his daughters, Karen Cook Perry (Joel) of Timberlake, NC and Connie Ann Douglas (Jeff) of Raleigh, NC; his brother Bruce Cook of Seneca, PA; and his grandchildren, Conner and Caroline Douglas. He is also survived by many nieces and nephews.
