He was predeceased by: his parents, Henry Cook and Yvonne Cook; and his brothers, Henry Gwineau Cook, James Wayne Cook, David Fairchild Cook and John Douglas Cook.
He is survived by: his wife Sara Cook; his children, Dana Leigh Smith (Tearl) and Brian Eric Cook (Grace Marfori); his grandchildren, Heather Lynn Hyman, Holly Yvonne Hyman, Tiffany Marie Hyman, Kristina Brianne Hyman, April Dawn Hyman and Dan Gallanosa; his great grandchildren, Evan, Emmarie, Gavin, Landyn, Shelby, Lucy, Silas and Rhys; and his sister Yvonne Eudy.
Memorial contributions may be given to Benton Heights Presbyterian Church Building Fund.
