He is survived by: his wife Ruth Sprouse; his children, Forrest Sprouse Jr. (Sarah) and Sue Ellen Sprouse; and his brothers, Albert M. Sprouse and Bernard H. Sprouse. He is also survived by 9 grandchildren.
