He is survived by: his wife Rachel Honeycutt Polk of Monroe; his children, Tim Polk (Jill Thomas Polk) of Monroe, Sheila Polk Simpson (Alan Simpson) of Monroe, Marcia Polk Hayes (Scott Hayes) of Rock Hill, SC and Monica Polk Mizzell (Derrick Mizzell) of Monroe; his siblings, Boyd Polk (Joan Purser Polk) of Monroe, Jerry Polk (Patsy Stegall Polk) of Monroe, Larry Polk (Delane Eudy Polk) of Monroe, Jeanette Baucom of Marshville and Louise Byrum (Dennis Byrum) of Monroe; and his sister-in-law Patsy Haigler Polk.
