He is survived by: his parent Etta Lail Cook; his grandchildren, Misty S. Carrol (David) of Kingsport, TN and Jonathan; his great grandchild Logan; his sister Jean Smith; and his siblings-in-law, Lois Cook, Mabel Carswell, David Johnson (Sue), Floyd Johnson (Beth) and Lloyd Costner.
