He was predeceased by : his aunt Maxine Mason of Ocracoke. He is survived by : his children, Isaac "Ikey" Delano O'Neal, Jr. and Lucy Louise O'Neal of Ocracoke; his uncle Lewis Tolleson, III of Florida; and his aunts, Judy Garrish of Ocracoke and Linda Gaskill of Ocracoke.

In lieu of flowers, memorial donations may be made to Life Saving Church – Ocracoke Assembly of God located at 459 Lighthouse Rd / PO Box 68, Ocracoke, NC 27960. All other donations may be made to the Ocracoke Interfaith Relief & Recovery Team "OIRRT" at PO Box 415, Ocracoke, NC 27960 which helped Ikey repair his home after the damage caused by Hurricane Dorian in 2019.
