He was predeceased by: his parents, Dean Sample and Rose Sample (Janicek); his stepfather Barry Christian; his brothers, Alton, Erwin and Dwayne Sample; his sister-in-law Karen Poland; his parents-in-law, Dennis and Jane Ballmer; and his grandson Dominic Dawn.
He is survived by: his former spouse Bobi Muff; his wife Cyndi Ballmer; his stepchild Corby; and his siblings, Bruce Sample (Bonnie) of Lincoln, Dan Sample (Judy) of Alliance, Aldon Sample (Mary) of Arthur, NE, Diane Arnold of Loveland, CO and Wendy Sogn (Justin) of Rapid City, SD. He is also survived by numerous nephews and nieces.
