She was predeceased by : her parents, Albert Prusha and Nellie Prusha (Bowman); her former life partner Vic; and her daughter Vicky Haroldson. She is survived by : her husband Victor 'Vic Zajicek; her daughters, Donna Hassebrock (Morris) and Patricia Forman (Robert); her grandsons, Michael Gemmel (Jill), Landon Gemmel (Susan), Bryan Forman (Marcie) and Brett Forman (Amber); her great grandchildren, Colton, Collin, Caden and Wesley; and her sister Donna Kos (Daryl).
