She was predeceased by : her parent Stephanie Dawn Young; and her grandmother Connie Young. She is survived by : her parent Stephanie Chavez of Lexington; her brothers, Erique Perez-Young (Dulce Flores) of Lexington and Nestor Perez-Young of Lexington; her niece Kiara; and her nephew Kalani of Lexington. She is also survived by numerous aunts, uncles, cousins, extended family and many, many friends.

A memorial has been established in Jazmin's honor, and kindly suggested to the family for later designation.
