He was predeceased by: his parent Mortimer O'Sullivan; his wife Barbara O'Sullivan (Adams); and his siblings, Rose Kretzschmar, William O'Sullivan, John and Sullivan.
He is survived by: his parent Rose O'Sullivan (Terry); his sons, Dennis "Jack" (Theresa O'Sullivan) of Boylston, MA and John "Jack"; his wife Linda O'Sullivan of Windham, NH; his grandchildren, Michael O'Sullivan, Matthew O'Sullivan of Boylston, Christie Quinlan (Garrett) of Nashua, NH and John O'Sullivan Jr. of Manchester, NH; his sister Margaret Riley of Lowell, MA; and also Europe. He is also survived by many nieces and nephews.
Memorial donations may be made in his name to the charity of one's choice.
