He was predeceased by: his parents, Frank C. Cook and Ruth Baldwin Cook; his daughter Sondra "Sondi" Cook Smith; and his brother G. Richard "Dick" Cook. He is survived by: his wife Sharon; his sister Francis "Carol" Cook; his children, Valerie Reynolds (James Reynolds) and Douglas Cook (Nicole Cook); and his grandchildren, Zakary, Makenzie Smith, Brooke Reynolds, Davinn, Logan and Lane Cook.

Arrangements will be held at Newbaker's Funeral Home, but attendance has been limited to immediate family due to the novel coronavirus pandemic.
