She was predeceased by: her husband Andrew J. Polascak Sr.; her brothers, Frederick Porter (Virginia), William Porter (Marie) and Robert Porter; and her parents, Frederick Porter and Dorothy Porter.
She is survived by: her children, Andrew Polascak Jr. (Julie) of Myrtle Beach, SC, William Polascak (Patricia), Jeffrey Polascak Sr., Mark Polascak (Eileen Hall) and Dawn Morton (Douglas); her grandchildren, Andrew Polascak III, Marissa Barnhill (Jordan), William Polascak Jr. (Melanie), Lisa Polascak (Laura Stackhouse), Jeffrey Polascak, Laura Polascak, Mark Polascak Jr., Jenna Lea Polascak and Elizabeth Morton; her great grandchildren, Donovan and Liam Polascak; and also, Tina, Dan Wolff, Sue, Eric Perkins, Andrea, Zach, Jack Braff, Cara and Chris Zimmerman.
