He was predeceased by: his parents, Helen Monchak and Stephen J. Monchak; and his sister Janet Monchak. He is survived by: his wife Elaine Monchak; his sons, Christopher Monchak and Gregory Monchak (Renee); his granddaughter Mia; and his daughter Karen Monchak (Chris Elsesser).
