She is survived by: her husband Richard De Forge; her children, Susan MacIntosh (David), Mary Ann Grubesich (Jacob), Diane Amorino (Fiancé Richard Domanski) and Carol Ann McCartney (Michael); her grandchildren, Raymond Perruzza (Pam), William Kukla (Joan), Nicholas Polkowski, Angelica Kukla (Brad), Samantha Amorino, Joshua Wagner and Joseph Amorino; and her children, Olivia Perruzza, Kylie Polkowski, Charlotte Kukla and William Kukla (Liam).
