He was predeceased by: his grandparents, Ida, George Szotak, Sr. and Latanzio Randazzo. He is survived by: his parents, George Szotak of Madison and Karin Szotak; his sisters, Allynn Szotak and Kiersten Szotak of Madison; his grandmother Beverly Randazzo; his godfather Steve Cherevko (Chris); his aunts, Auntie Arlena Boyle (Chip), Elinore Payne (Chuck) and Patti Ascolese; Nick Thompson of Randolph.
In lieu of flowers, the family has requested charitable contributions may be made in CJ's memory to National Alliance on Mental Illness or to the ASPCA.
