She was predeceased by: her husband Thomas Flach; and her daughter Mary Jo Flach. She is survived by: her daughter Elaine Caccavale; her sister Mary Snyder; her grandchildren, Mark Caccavale, Michael Caccavale, Victoria Flach and Tanner Flach; her great grandchildren, Chase Caccavale, Matthew Flach and Jacob Flach; and her nieces, Kathy Campion, Karen Miezejewski, Debra Lynch and Denise O'Brien.
In lieu of flowers, the family requests that donations may be made in Helen's memory to the Madison Volunteer Ambulance Corps, 29 Prospect St., Madison, NJ 07940.
