He was predeceased by: his parents, Ferdinand Slavkovsky and Patricia Slavkovsky. He is survived by: his siblings, William Slavkovsky and Susan (Michael Gull); his nieces, Christyn Slavkovsky and Rachel Oglivie; and his nephew Shawn Slavkovsky.

In Lieu of flowers donations can be made to the humane society.
