He was predeceased by: his parents, Charles Lazorchak and Anna Lazorchak (Pekar); and his sister Jean Ann Harvey. He is survived by: his wife Donna Nina; his sons, Charles A. Lazarchak (Jan) of Walnutport and Adam S. Lazarchak (Linnea) of Nazareth; and his granddaughters, Briana, Meghan and Sarah.
