He was predeceased by: his wife Estelle R. Fonzlow; his daughter Sharon R. Costantini; his great grandchildren, Rebecca, Rachael, Ryan and Owen; and also, Richard M. Costantini, David Costantini, Tracie Costantini, Lisa Groewa Bradley and Brian Bradley.

He is survived by: his daughter Faye Fonzlow.
