He was predeceased by: his parents, James Forshay and Anna Toth Forshay; his former wife Terry Lynn Burke Forshay; and his brothers, John and James Forshay.
He is survived by: his wife Amy Hines; his daughters, Karen Fischer (Forshay) and Jacquelyn Wilson (Forshay); his grandchildren, Robert Fischer, Andrew and Hazel Wilson; and his siblings, Richard Forshay, Mary Ann Huebner (Forshay), Barbara Ierardi (Forshay), Virginia Costa (Forshay) and Ruthann Weiss (Forshay). He is also survived by many nieces and nephews.
