He is survived by: his mother Judith Dake Forward Sr. On; his wife Nicole Hutchison; his daughters, Danah Forward of Salamanca, Kenze Tingue of Great Valley and Mackayla Forward of Salamanca; his grandchildren, Ryan Szary, Greyson Forward, Mason, Ace and Colin Forward; and his sisters, Tammy Forward (John Woodard) of Franklinville and Carol Forward (Brad Rathburn) of Franklinville.
