He was predeceased by: his parents, Carlton W. Forsythe, Sr. and Mary M. Forsythe (Wood); and his siblings, Roy and Carlene Forsythe. He is survived by: his wife Beverly Jesmer; his sons, Jeff (Clover Forsythe), Steven (Dawn Forsythe), David (Karen Forsythe) and Carlton Forsythe, III; his sister-in-law Karen Forsythe; and his grandchildren, Adriane, Steven Jr., Zachary, Jake, Alexandra, Olivia and Josiah. He is also survived by 5 great-grandchildren.
