We are sad to announce that on November 17, 2018, at the age of 91, Frances Bermudez (Herkimer, New York), born in Utica, New York passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parents, James Bouzan and Alice Perry; and her stepfather Oscar Perry. She is survived by: her spouses, Arthur F. Kozakowski and Max Bermudez; her daughter-in-law Thomas Kozakowski of Endicott; her daughter Patricia Sullivan (Mike Dychko) of Herkimer; her grandchildren, Josh Mellander, Samantha Kozakowski, Brandon Kozakowski, David Kozakowski, Erin Kozakowski, Sean Sullivan and Ryan Sullivan; her great grandchildren, Arianna Kozakowski, Xoey Kozakowski and Caroline Mellander; her godchildren, Barb Suttmeier Burdick, Rocco Luppino, Joann Catalina, Daryl Frey, Peter Trzepacz and John Wezalis (Josh); and her close friend Maria Hunt. She is also survived by several nieces, nephews and cousins.
