She is survived by: her boyfriend Michael Crandall; her children, Jessica Perkins (Chris) of Tioga, ND, Anthony Derby (Kelly) of Laverne, AL, David Derby (Rachael) of Rochester, NY and Sierra Derby of Concord, NC; her father Dave Chappell I of Bloomfield, NY; her siblings, David Chappell II (Kate) of Bloomfield, NY, Christine Nudd (Chris) of Clifton Springs, NY, Chrystal Chappell (Calvin) of Newark, NY and Robin Smith (Dave); her grandchildren, Jailynn and Audry Perkins; and her grandmother Rina Chappell of Canandaigua, NY. She is also survived by several nieces, nephews, aunts, uncles and many friends.
