He is survived by: his parent Helen Mager (Zadorozny); his wife Michelle Hura Mager; his children, Stephanie Lara (Eduardo) of Brooklyn, NY and Lauren Sullivan (Conner) of Newburgh, NY; his sibling Myron J. Mager (Sharon); his siblings-in-law, Melissa Harpst and Michael E. Hura (Olga); and his sibling's children, Melissa Clark, Rebecca Johns, Morgan Cornell, Zachary Harpst, Garrett Harpst, Michael S. Hura, Mitchell Hura, Emily Hura and Marissa Hura.
In lieu of flowers, memorial donations in Jack's memory can be made out to the American Heart Association at 301 Manchester Rd # 105, Poughkeepsie, NY 12603.
