She was predeceased by: her parents, Gordon Chapin and Noreen Smith Chapin; and her brothers, Rusty Chapin and David Chapin. She is survived by: her husband Jack L. Cyzeski; her children, Tammy S. Ketchner (Mike) and Chris J. Cyzeski; her grandchildren, Taylor C. Ketchner, Marissa C. Ketchner, Madisyn N. Ketchner and Ryan M. Ketchner; her great grandchild Asher; and her sister Judith McHenry of Cuba. She is also survived by nieces and nephews.

In lieu of flowers, memorials may be made to the United Pentecostal Church, 1100 Homer St., Olean, NY 14760.
