He is survived by: his parents, John Formica and Susan Ficalora; his stepmother Patricia Formica; his sisters, Angela (Dan) and Natalya; his grandparents, Filippo and Irene; his aunts and uncles, Salvatore (Susan), Anthony (Barbara), Nancy (Bill), Bobby and Lisa (Steve); and his cousins, Nicholas, Thomas (Shannon), Irene (Kyle), Johnny, Madeline, Filippo, Emma, Anthony and Angelo.
Visitation was held on Wednesday, November 12th 2025 from 2:00 PM to 4:00 PM, from 7:00 PM to 9:00 PM and on Thursday, November 13th 2025 from 5:00 PM to 7:00 PM at the Arthur F. White Funeral Home (315 Conklin St, Farmingdale, NY 11735). A funeral Mass will be held on Friday, November 14th 2025 at 10:00 AM at the St. Martin of Tours RC Church (220 Central Ave, Bethpage, NY 11714).
The family asks on Philip's behalf that in lieu of flowers, a donation be made to either the Make a Wish Foundation or the Ronald McDonald House.
