He was loved and cherished by many people including: his children, Patricia Kozak (Robert) and Forrest Cousens II (Faith); and his grandchildren, Blair Reynolds (Kozak), Tara Lindsay (Kozak), Matthew Cousens and Amie Fredericks (Cousens). He was also cherished by nine great-grandchildren.
In lieu of flowers, the family requests donations in Forrest's memory to The Poughkeepsie Reformed Church (in memo section of check, please write "Organ Fund").
