Phillip R. Cook Obituary

With heavy hearts, we announce the death of Phillip R. Cook of West Winfield, New York, born in Ilion, New York, who passed away on July 28, 2020 at the age of 68. Leave a sympathy message to the family on the memorial page of Phillip R. Cook to pay them a last tribute.

He was predeceased by : his parents, Milton Cook and Garnet Cook (Perrigo); and his granddaughter Courtney. He is survived by : his wife Linda Slack; his children, Shelly Walther (Chris) of Sprakers, NY, Patrick Cook, Chris Cook, Scott Gay, Jason Gay (Jodi) and Andy Cook; his grandchildren, Amanda (Alando), Nicholas (Cerina), Emily, Luke, Matthew, Stephanie, Skyler, Mackenzie, Nathan (Amanda), Megan (Cody), Katie, Rebecca and Johann; and his great grandchildren, Azlyn, Zoey, Everleigh, Levi, Adalyn and Leo. He is also survived by several nieces, nephews and cousins.

In accordance with the National Funeral Directors Association and the CDC, masks must be worn and proper social distancing practiced.
