He is survived by: his mother Patricia Kolakowski; his siblings, Kevin Kolakowski, Keith Kolakowski and Kelly Faczan; his siblings-in-law, Fumi, Analia and Roland; his nieces and nephews, Alex, Keira, Keanu, Liliana and Daniel; and his pet Jack. He is also survived by his nieces' or nephews.
